Promoting "Integration of Bio-Medical Things (IoBMT)"/CRDS-FY2018-SP-06
In recent years, the amount of data produced in the life science / clinical medicine field is dramatically increasing. As a result, a method to analyze vast amounts of data and lead to new discoveries has greatly advanced, and "data research" is now directly linked to the impact of research and development results in the field of life science and clinical medicine. In addition, research has been promoted mainly on model organisms such as mice, but shifts to "human research" are accelerating. Furthermore, the digitization of medical information has greatly progressed, the collection and analysis of a large amount of medical data generated in daily practice nationwide finally became possible finally, and the utilization for medical administration decision making support has finally started. "Human research" and "data research" are pillars of future life science and clinical medicine research. In addition, social and economic impacts such as improvement of QOL of the people, strengthening of industrial competitiveness, decision support of medical administration, etc. are great. However, the movement of Japan is extremely slow as compared with the West.
In order to improve the international presence in the field of life science and clinical medicine in Japan, strategic acceleration of "human research" and "data research" is an urgent task. This strategy proposal aims to realize individual prevention / foresight medicine which is the form of new medical treatment from now by setting important research and development / foundation theme group essential for that. We defined these series of directions as "Integration of Bio-Medical Things (IoBMT)" and compiled this strategy proposal.

As a result of examination from the viewpoints of research and technology trends, our strengths and weaknesses, existing domestic efforts and social needs, nine themes to be promoted in the future were considered. In this strategy proposal, we will detail the three topics of particular importance among them. For the remaining six themes, a separate strategic proposal or survey report will be published (some of which have been published).

The outline of 1) to 3) proposing details in this strategy proposal is as follows.
1) Construction of 'Integrated Innovation Base for Human Disease Research (tentative name)' - Review of the way university / National Research and Development Agency / related hospitals are -
The implementation system responsible for medical research in Japan needs to regain vitality through bold reform. In this strategy proposal, we propose "Human disease research integration innovation base". At this site, headquarter organizations with strong authority oversee research institutions / hospitals and common infrastructure research and support organizations under their umbrella. A bold reform plan will be raised from time to time by a third party organization, and headquarter organizations will put it into practice. The base enables bold activities with no clue by acquiring diverse funds of public and private sectors. We conduct legal support (such as installation law) for the entire base including these management systems. Specific research and support activities to be carried out within this center include collecting, structuring and utilizing health and medical data, developing a foundation for utilization, deeply understanding diseases, promoting cross-cutting research in a balanced manner, promoting medical technology evaluation (Promotion of regulatory science), support for clinical research and clinical trials, active implementation of doctor-initiated clinical trials, acceleration of industry-academia cooperation / international collaboration, securing of human research and related "data research" human resources, next generation Promotion of epidemiological research, improvement of research equipment platform (core facility), development of bio-banks, maintenance of biobanks, humanities society × promotion of medical science and medicine (support for improvement of basic law on disease control, study of public participation type research and development system) And so on. By practicing these, it will be expected to lead the world as NIH research and development base for human diseases concurrently with NIH.
2) Structuring disease / medicine "knowledge" / Construction of next generation electronic medical record system
In our country, the current analysis target is limited to data in easy-to-analyze format such as the receipt, but in order to continue to create new value in the medium to long term, it is necessary to expand the range of data to be analyzed is there. Because Japan has a health and medical data group that has been deceased because it is a form hard to analyze what contains extremely high-quality information, the challenge for analyzing them is important. Also, at clinical sites where data is generated, minimization of physician's burden associated with recording information (including collection of new types of information such as doctor's idea record) and maximization of merit (provision of medical support system etc.), It is also important to develop a new system that can be called a "next generation electronic medical record system". By accelerating the implementation of the system on the clinical site, research on health and medical data in Japan will be greatly revitalized and it will be possible to continue to create new value in many ways in the short term even in the short term.
3) Whole cell life and medical science in humans
Since the 21st century, new developments are seen in genome related technologies that have strongly led the life science and clinical medicine research. It is a direction to try to analyze all cells of the whole body with one cell level refinement. Although it is technically immature at the moment, it is considered to be a powerful experimental tool which all researchers of life science and clinical medicine use for daily research. Since there are many technologies (such as imaging technologies) that Japan has strengths in the components of this technology, the urgent task is to build and utilize the technological foundation that makes good use of them.
